Parent Pick-Up

For the safety of all children, a child will only be released to a parent who appears on the child’s birth certificate or is listed on the Pick-Up Authorization Form. Any change that needs to be made to this pick-up list can be done by submitting the completed Notice of Pick-Up Change.

Any parent or guardian who does NOT want someone listed on the birth certificate to be able to pick up their child will have to submit court documentation that the biological parent has lost their parental rights.
